| Your pet is coming in for a complete dental prophlaxis. |

| If during the dental procedure the doctor finds a loose a compromised tooth a dental x-ray may be
recommended to determine the viability of the tooth. Should a tooth have an exccessively deep periodontal pocket, the doctor
may recommend filling the pocket with Doxirobe (an antibiotic). In extreme cases where a tooth is beyond saving, the doctor may
recommend extraction. To prolong the period between dentals and to prolong the life of your pet's teeth, we recommend the
application of OraVet as a barrier sealant to aid in prevention of dental plaque and calculus. Please elect the following

| While under anesthesia, if the doctor find the above or other conditions necessitating treatment
or diagnostics: |

| Like you, our greatest concern is the wellbeing of your pet. Before putting your pet under anesthesia,
we will perform a full physical examination. However, many conditions including disorders of the liver, kidneys and/or blood are
undetected unless blood testing is performed. Such tests are especially important before any kind of surgery and are highly recommended.
The tests we recommend are similar to and equally as important as those your own physician would run if you were to undergo anesthesia.
It is important to understand that a pre-anesthetic profile does not guarantee the absence of anesthetic complications. It may,
however, greatly reduce the risk of complications as well as identify medical conditions that could require further treatment. |
| Our laboratory is fully equipped and staffed to perform these tests. Results will be immediately
available to examine before anesthesia and/or surgery. |
